Medical Records / Release of Information

iPSYCHS is required to keep appropriate records of the psychotherapy services we provide for patients utilizing health insurance, which is time-consuming. Often, in therapy, you will see your psychotherapist taking notes because we like to reflect on what the client has discussed, track problems and reoccurring themes, and to continuously evaluate the client’s needs. Your records are maintained in a secure location in your psychotherapist’s office. If there are forms, they are maintained in a locked file cabinet. As for online data, we have an electronic health records (EHR) system that is HIPAA compliant. iPSYCHS’ Patient Portal (through Simple Practice) keeps records noting that you were here, your reasons for seeking therapy, the goals and progress we set for treatment, your diagnosis, topics we discussed, your medical, social, and treatment history, records we receive from other providers, copies of records iPSYCHS send to others, and your billing records. We also track communication that is outside of session. Please be mindful that the therapist’s full notes are their personal property and verbatim notes do not have to be released.

 

If you would like to request a copy of your patient records or release to a 3rd party (such as spouse or other medical provider), a Release of Information form must be completed. Please complete the following form and either scan & upload to your Simple Practice portal account for the therapist or send via email to office@ipsychs.com.
